Locum Consultant Physician - Special Interest: Acute Medicine and Diabetes and Endocrinology

Full Time (10 Sessions plus 1 temporary addiitonal session)

Duration: up to 6 months

Based at Ysbyty Ystrad Fawr, Caerphilly, SE Wales

This is the replacement post for one of our Consultant Physicians. The post is based in Ysbyty Ystrad Fawr (YYF), Caerphilly, in Aneurin Bevan University Health Board. This replacement job is being advertised following a Health Board-wide service review including a comprehensive reconfiguration of service for the Acute Medicine within the Unscheduled Care Division and specifically at YYF.

The post holder will also be part of acute General Medicine on call (currently 1:9). The aim is to provide a spectrum of flexible and responsive services for the Medical Directorate as well as good medical care for all age groups across the Health Board encompassing:

  1. A comprehensive Acute Medicine Service for the Borough of Caerphilly
  2. Support the running of the Medical Assessment Unit and participate in the Adult Medicine take 7 days a week
  3. Acute inpatient assessment and treatment
  4. Development and clinical delivery of comprehensive Medical and Diabetes services at Ysbyty Ystrad Fawr, Caerphilly Borough.
  5. Hospital based General Medical and Diabetes Services
  6. General and specialist outpatient clinics
  7. Support and training of medical staff, nurses and other members of the MDT
  8. To provide effective leadership to multidisciplinary staff members 
  9. To develop teaching and research with the organisation

The consultant on call rota is 1 in 9. The average number of medical assessments in MAU is approximately 30 per 24 hour period. In addition we have ambulatory care unit (ACU) which has only 4-5 patients/day. There is a selective take as there is no Accident and Emergency attached to this unit but there is a Minor Injury unit. The medical take is selective with clearly agreed inclusion criterion. At present there is an integrated on call rota for consultants in adult medicine with prospective cover.The consultant appointed to this post will be doing three sessions in MAU (8 am to 8 pm). Morning post take ward round usually finish by 10 am. There will be 4-5 ambulatory patients coming in the afternoon. There is another clinical fellow (Registrar grade) who also supports consultant in the ACU and is in addition to the on-call team during the day (1 middle grade, one Foundation year/GPVTS and one FY/GPVTS doing twilight shift from 2 pm to midnight). MAU is also supported by ANP both during the day and night.Therefore, an extra session has been included in the job plan as the consultant will be covering on call – 5 pm until 8 pm - which is predictable work.The job plan will be reviewed regularly to ensure that new consultant gets appropriate free time for professional development and CPD activities. The on call team which is resident covers both admissions and inpatients out of hours. This includes a Registrar, CF/FY2/GPTS as well as Nurse Practitioners. There is always a resident medical doctor (middle grade) and nurse practitioner covering the take as well as the wards when on call.There will be 3 sessions for SPA which needs to be justified and one session for administrative work.
