GENERAL DESCRIPTION OF THE POST

Applications are invited for an Oncoplastic Training Fellow post in the Breast Surgery Unit for 6 months. The appointee will be at Senior Registrar level and have gained significant experience in Breast Surgery and have some Oncoplastic experience. The new appointee will be part of the Breast Surgery Team and involved in the whole patient pathway. We currently have 6 Consultant Breast Surgeons, 5 Consultant Breast Radiologists and 3 Consultant Breast Radiographers. We have three dedicated Oncoplastic Breast Surgeons who will provide focussed Oncoplastic training for the successful applicant. The unit runs live courses for partial breast reconstruction and local perforator flap based reconstruction.

The department is complemented by 6 breast care nurses, 1 Specialist Registrar, a Trust Grade, an ISTP fellow and an SHO. There is also a Surgical Care Practitioner who is trained to do minor surgical procedures and see new and follow-up breast patients in the clinic.

The breast team at SWBH works flexibly as a team and on an annualised contract. All outpatient clinics and theatre lists have colleague cover built in and the successful applicant will be expected to be part of a team delivering breast services rather than have fixed commitments. The Trust and the Breast Unit are working towards establishing an annualised job plan for all permanent staff.

You may be expected to undertake some general surgery commitments and participate in a hernia service. The candidate is not expected to participate in the general surgery on call rota.

The breast unit at Sandwell and West Birmingham NHS Trust is the hub for the SWBH breast screening unit serving a screening population of over 170000 women across the North and West of the region.

The unit is actively involved with Clinical Trials and research and has established links with the University of Birmingham and University Hospitals Birmingham.  The successful candidate will be expected to help with recruitment of patients to Clinical Trials and take part in research.

The post will initially be based primarily at City Hospital. However the Trust will be reconfiguring its services in preparation for the implementation of the Right Care Right Here Programme and the post holder may therefore be required to work at any of the Trust’s sites.

Facilities and Resources of the Breast Surgery Department.

The Department:

The Breast Unit is based at the Birmingham Treatment Centre which is the main ambulatory care centre for Sandwell and West Birmingham NHS Trust. This is a purpose built facility with Breast Surgery, Imaging, and Nursing all housed in this building adjacent to each other.

The patients undergoing surgical procedures are admitted as day cases in the Adult Surgical Unit in the Birmingham Treatment Centre. Nuclear Medicine Department is based on the same site in the main spine of the hospital and injection for sentinel node biopsy is carried out in the Breast Unit on the day of surgery.  Almost all the breast surgery is done as a day case procedure and the newly appointed doctor will be expected to facilitate this practice.

The combined annual workload is more than 5500 new referrals per year with approximately 500 newly diagnosed breast cancers treated per year.

Within the Trust the Breast Unit has won a prestigious Beacon Service award. Nationally we were previous winners of the HSJ Efficiency award. We pride ourselves on excellent team working and had this recognised nationally by being a previous Nursing Times Team of the Year finalist. We were pioneers of the day case mastectomy pathway which subsequently became a national standard for best practice.

Clinics:

MDM’s:

Separate symptomatic and screening MDM’s.

Imaging:

The breast imaging team consists of 5 Consultant Radiologists. In addition there are 3 Consultant Radiographers who perform film reading and ultrasound scans, and are fully trained to do interventional procedures i.e.,  guided core biopsies, insertion of wires and vacuum-assisted biopsies.

Consultant radiology input is available in all rapid access diagnostic clinics where there is immediate imaging and reporting. Where indicated guided biopsies with the benefit of the ultrasound and digital stereotactic equipment can be done at the same visit.  We have routine VAB available and Tomosynthesis.

The breast unit participates in the NHS Breast Screening Programme and is part of the City, Sandwell and Walsall Breast Screening consortium which invites over 55,000 women each year for breast screening and covers a population in excess of 170,000 women.

Nuclear Medicine on site at City Hospital provides facilities for investigative imaging and isotope for sentinel node biopsy which are undertaken at the Birmingham Treatment Centre.

MAIN DUTIES OF THE POST

The key duties of this post are:

  • To work closely with the consultants in the management of patients undergoing breast or general procedures from the pre-operative outpatient assessment through to the operating theatre and post-operative care both on the ward and in clinic.
  • To provide support for all clinical activity including the day-to-day management of patients undergoing breast procedures, as well as some general surgical and plastics cases.
  • To provide and develop further with clinical colleagues, Breast surgical services for the Trust in collaboration with colleagues in all disciplines including primary and tertiary care, were appropriate, to provide an integrated service for patients.
  • To participate in provision of Breast and General Surgery services and to support the Consultant led service, including outpatients, pre-admissions and theatre.
  • To participate in the development of Breast surgical services, treatment protocols and guidelines and the greater knowledge of the management of patients with such conditions.
  • To develop and maintain collaborative relationships with medical colleagues in other specialties and participate in regular clinical meetings and other postgraduate activities.
  • To develop and maintain good communications with appropriate outside agencies.
  • To demonstrate a firm commitment to the principles of clinical governance, including:
  • Developing and maintaining appropriate systems and practice to ensure effective clinical audit in the Breast Unit.
  • Attending and contributing to the Trust’s Clinical Governance Programme.
  • Participating in the Trust’s Clinical Incident Reporting system.
  • Developing a programme of personal continuing professional education and development, within available resources and within the workload and priorities of the service, as agreed with the Clinical Director and Group Director and in accordance with the Royal College requirements.  The Trust will provide the necessary support for this.
  • To ensure own practice is up-to-date; this will include taking responsibility for own clinical professional development and participating in the Trust’s Performance and Development Review system.
  • To share responsibility for data protection arising out of the use of computers.
  • To comply with all relevant Trust Policies and Procedures.

    The duties are subject to the post holder’s acceptance that other duties may be required in occasional emergencies and unforeseen circumstances.
  • There are no on call commitments.
