Unscheduled care is undergoing a redesign as part of an improved Hospital wide service delivery model. Central to which is an Acute Floor across which specialists in Emergency Medicine and Acute Medicine will work collaboratively providing a Consultant led service for unselected acute admissions. With a growing elderly population and increasing recognition of the need for doctors with generalist skills, we are looking to recruit an enthusiastic, flexible, high calibre physician with capability in Acute Medicine.

Currently acute services are arranged in a traditional model of an Emergency Department and an Emergency Admissions Unit (EAU), with patients referred to EAU either from ED or primary care. Additional recruitment has been committed to provide a total of 6 Emergency Medicine Consultants and 3 Acute Medicine Consultants to allow a for 7 day Consultant led Acute admission service to be instigated – known as the Acute floor. The Acute floor will incorporate the Emergency Department and a 22 bedded short stay Acute ward. Patients with a wide variety of emergency presentations admitted to the acute ward will be managed conjointly by the Emergency and Acute physicians, with input from specialty teams (Cardiology, Gen. Surgery, ENT, Orthopaedics etc.) as required.

In addition to the Acute Floor, we are expanding Ambulatory Care services, rapid access clinics and follow-up clinics in order to offer alternatives to hospital admission when safe to do so, and facilitate early discharge alongside recent successful community innovations. These include a Rapid Response and Reablement team staffed by nurses and allied health professionals delivering care at home, including intravenous antibiotics, Intermediate Care beds at a local nursing home, and a Respiratory community nursing team.

The aim of this post is to provide senior clinical decision making on a daily basis to the acute floor, help develop the ambulatory care model including ambulatory clinics and to enhance links with the Emergency Department.

There will also be opportunities for specialist clinics according to the interests of the successful applicant.
