You are invited to apply for the first of 3 planned new consultant posts to be advertised over the next few months to join the department of elderly care at Royal Hampshire County Hospital, Winchester. The team comprises a main body of 7 consultants, with orthogeriatric liaison provided by an additional
consultant. Further clinic sessions are provided by another 2 consultants and 1 GP.

Following the successful establishment of our acute frailty unit, the post advertised begins the recruitment for the next phase in our long-term frailty project. We plan to establish a frailty team consisting of a consultant, junior doctor, specialist nurse and occupational therapist based in the emergency department for 50 hours per week. This brings specialist geriatric care as close to the front door as possible. An additional aim is to enhance the current rapid access clinic service to facilitate same day or next day assessment, so it can provide a genuine alternative to admission for frail older patients.

The department provides inpatient care for around 70 patients on 2 acute wards and an acute frailty unit. The latter is embedded within the medical admissions ward and accommodates all frail medical admissions to the hospital.

It is envisaged that the new post will predominantly rotate between the emergency department frailty team, acute frailty unit and acute elderly care wards, with input into the rapid access clinics at all stages, though a preference for one part of the pathway could be accommodated.

Our long-term vision is to progressively reconfigure the service along the lines of stroke medicine. The ultimate aim being for frail medical admissions to enter a specialist geriatric infrastructure at the front door and remain within one until the point of discharge. With further successful recruitment this  service will be supplemented by access to same day or next day ambulatory care / rapid access clinic assessment for patients not necessarily requiring admission. Patients would also have access to an early supported discharge service when required. This vision is underpinned by three additional ideals of care: minimising ward moves, minimising multiple clinical assessments, and minimising delays to discharge in patients no longer needing inpatient services.

On-call commitments are on the acute medicine rota. They currently involve a medical admitting take approximately 1:14. (Additional 2PAs for on call duties). There is a weekend ward round rota approximately 1:12 for morning ward round seeing targeted patients on the medical wards with time off in lieu.

The applicant should hold a CCT in General Internal Medicine and Geriatric Medicine or be with 6 months of completion.
