Here at Yeovil, we are currently reconfiguring and expanding our Geriatric Medicine department in order to meet the needs of an increasingly frail and complex population. South Somerset and West Dorset have a large retired population who are increasingly living longer and requiring highly coordinated care both in the hospital and in the community. As a Vanguard site, we already have the innovative Symphony project which seeks to address the needs of our most complex patients and aims to prevent admissions by meeting needs in the community before the patient reaches crisis point. However for those patients who do present as emergencies, we recognise we need to reconfigure our services to provide consistent geriatrician led MDT input at the earliest possible stage and throughout the admission. Over the past few years, we have developed the Frail Older Person’s Assessment unit at YDH which provides rapid access assessment to patients referred from GPs, paramedics and ED. This has been very successful at reducing admissions and providing MDT assessments but we now would like to extend our service to meet the needs of all frail patients presenting as emergencies to the acute trust.

We are looking to recruit enthusiastic and highly motivated geriatricians to enable us to provide the level of care we know our patients deserve. You would join a small but enthusiastic team who are very keen to recruit and work with new colleagues. We are also looking to recruit Specialty Doctors and Advanced Nurse Practitioners to build the team. Any subspecialty interest would be welcome although our focus currently is on acute frailty and meeting the needs of frail older people within the emergency department and acute medical wards. We are looking to establish a short stay frailty unit to enable us to minimise the time older people spend in hospital but also to provide comprehensive geriatric assessment so that they leave with an adequate management plan. We are also looking to extend our Orthogeriatric service to cover all older people with fractures as well as providing a surgical liaison service eventually.
