What is Epsom Health and Care (EHC)?
Epsom Health and Care is an innovative alliance between Epsom and St Helier University Hospital NHS Trust (who provide hospital-based acute services services), Surrey County Council (who provide adult social care), CSH Surrey (a social enterprise who provide NHS community health care and therapy services), and 19 local GP practices which serve the Epsom Locality of Surrey Downs CCG area. Our vision is to create new ways of working across organisational boundaries and transform people’s experiences of care by being a leading provider of integrated health and social care services.
Our services are provided by people from across health and social care working together as a single team, including doctors, nurses, physiotherapists, occupational therapists, community matrons and re-ablement assistants.
We are currently in an exciting phase of expansion. Since its inception in 2016, EHC has focused on providing care for people with acute and complex needs over the age of 65. However, the long term vision is to provide a full population health integrated model of care.
EHC Post Acute Unit
EHC are developing a new model of care transforming post-acute and community rehabilitation care for people whose needs can be met outside of an acute setting. We know that the longer people, particularly older people, remain in an acute hospital the lower their potential for rehabilitation and recovery of independence, due to the potential for muscle wastage and loss of confidence.
EHC are therefore developing a post-acute unit that will provide a short term bedded facility, as well as intensive therapy, re-ablement and care planning support offering a ho-listic approach to care and allowing them to maintain their level of health and independence and move to a more appropriate set setting of care as quickly as possible.
This unit will be closely integrated with New Epsom and Ewell Cottage Hospital NEECH with new aligned ways of working being developed in partnership which includes shared care plans, use of an integrated IT system and streamlined pathways between the units By integrating in this way we aim to ensure that people have continuity of care of as their recovery progresses, allowing a seamless journey from high intensity acute services through to community rehabilitation, and on-going coordination within the primary, community and voluntary services which will support them on return to their place of residence.
What are we looking for?
We are looking for dynamic GP to provide the management and leadership to the post-acute unit playing a key role in its development. You will have excellent organisational and time management skills and be passionate about delivering holistic integrated care that has the patient at its centre. You will relish developing new ways of working to im-prove quality and adding to the “one team” culture.
