There has never been a more exciting time to join us. Tameside and Glossop Integrated Care NHS Foundation Trust (T&G IC) will provide the foundation for one of the first ICOs in England, bringing together a wide range of health and social care services for the benefit of local people. The ICO will manage a person’s entire care in the future, by linking up hospital services with community care and mental health services as well as working closely with GPs.

We expect that, to fully transform these services, will take between three and five years. This is a unique opportunity for Tameside and Glossop to be at the forefront of developing more joined-up integrated care; and we are absolutely committed to getting it right. To get it right, we need the right, highly skilled patient focused workforce throughout the whole process.

We are looking for a dynamic and motivated individual to join our Palliative Medicine service at Tameside. The hospital team currently consists of 2.4 WTE specialist nurses, providing a 7 day service. The Trust is intending to recruit to a part-time sub-consultant post in the near future. The hospital Specialist Palliative Care team have strong working relationships with the Community Macmillan team and the clinical team bases at the local hospice.

These multidisciplinary relationships provide excellent means to develop protocols and pathways to collectively enhance patient care across the locality to the levels proposed by the National Ambitions for Palliative and End of Life framework.

The post holder will be required to drive improvement in the service as required by patients, staff and the government, and to promote best practice in the management of all cases.

Tameside & Glossop Integrated Care NHS Foundation Trust provides hospital and community services for children and adults across Tameside and Glossop. The Trust is situated at the foot of the Pennines, eight miles to the east of Manchester and serves a population of approximately 250,000. The population is concentrated in the largely industrialised areas of the eight townships of Tameside, which comprise Tameside Metropolitan Borough Council. However, Glossop with its population of approximately 28,000 is part of Derbyshire High Peak District Council, which provides the challenges of a more rural community.

The Trust is nestled at the foot of the Pennines, eight miles to the east of Manchester and serves a population concentrated in the largely industrialised areas of the eight townships of Tameside, which comprise Tameside Metropolitan Borough Council. This is in addition to Glossop, a market town within the Borough of High Peak in Derbyshire. The Trust is located within a close proximity to the M60 motorway network and is also closely located to the towns of Barnsley, Huddersfield and the city Leeds
