The Healthcare Safety Investigation Branch (HSIB)
was established by the Government in 2016. Headed
by the Chief Investigator, it is tasked with carrying out
independent safety investigations across the healthcare
system in England, and became operational from April
2017. HSIB is hosted by NHSI but will maintain its
independence both in the conduct and the reporting of its

The purpose of HSIB is to improve patient safety
across the healthcare sector, by carrying out expert
investigations to determine the causes of accidents and
serious incidents, and to make safety recommendations
intended to prevent recurrence. It is not to apportion
blame or liability. HSIB will also seek to improve
investigation practice across the system.

HSIB have taken responsibility for setting up maternity
investigations from April 2018 onwards and this national
programme will aim to reduce stillbirth, neonatal
death and severe brain injured babies and maternal
deaths. HSIB will undertake the most serious maternity
investigations using our independent, impartial and
standardised process that does not apportion blame or
responsibility. HSIB will recruit 100 plus investigators
over 2018 and the clinical director (maternity investigations) will be integral in providing strategic clinical leadership and expert advice to support the maternity programme.

We have an exciting and unique opportunity for
a clinical director 3 days/week (minimum) We invite applicationsfrom senior experienced doctors in obstetrics or a
relevant discipline. This post is offered on a part
time, fixed term or secondment basis enabling the
candidate to maintain part time clinical practice within
their speciality. We can accommodate other working
patterns dependent on availability.
