Specialty Doctor in Intermediate Care
An exciting opportunity to be part of the well established Community Resource Team at Blaenau Gwent is now available. This is a non-training, permanent post in an intermediate care setting based on Holistic Frailty care model of the Gwent Frailty Programme. You will experience and deliver Comprehensive care for the frail elderly working at the interface between Hospital and Primary care.
Blaenau Gwent CRT is a fully integrated team of Health & Social care professionals. Your role would be to provide Medical input for the team in delivering acute assessments and Hospital at home interventions in the community avoiding unnecessary Hospital admissions of Frail individuals. You will be joining 1 Intermediate Care Consultant and a Rapid Nursing Team.
Free parking is available across all Aneurin Bevan University Health Board Sites.

This position involves Regulated Activity with adults as defined by the Safeguarding Vulnerable Groups Act (amended by the Protection of Freedoms Act 2012) and the following checks will be undertaken following any conditional offer:
