Collaborative working for South Yorkshire and Bassetlaw Stroke Physicians

This is an exciting opportunity to be part of the new team to deliver the SYB regional model to provide high quality stroke care for patients. The Care of the Elderly Departments at Barnsley Hospital NHS Foundation Trust and The Rotherham NHS Trust are seeking to appoint Consultant Physician in Stroke Medicine and Hyper acute Stroke Medicine. These posts are replacement posts and an exciting opportunity to join dynamic Care of the Elderly and Stroke teams. The post is to aid further development of Acute Stroke Services and is a 10 PA post with 1 Additional Programme Activity (this will be reviewed annual at Job Plan Meeting).

Stroke is identified as a priority area for the South Yorkshire and Bassetlaw Accountable Care System that is recognised nationally as a leading Accountable Care System.

South Yorkshire and Bassetlaw are embarking on the development of a new model of Hyper Acute Stroke Care.

To deliver improved outcomes and best practice care the new model includes the development of a Managed Clinical Network for Stroke across South Yorkshire and Bassetlaw.

The network will have an operational focus and look across the whole pathway. It will provide an opportunity for clinicians to network with peers, share best practice and be part of an operational delivery network to implement the new model of hyper acute stroke care and collaboratively lead improvements in all aspects of stroke care.

The new model consolidates hyper acute stroke care in South Yorkshire and Bassetlaw at Doncaster Royal Infirmary and Sheffield Teaching Hospital. Chesterfield hospital will continue to provide hyper acute stroke care and some patients (predominantly those living in Barnsley) will also receive hyper acute care in Pinderfields hospital in Wakefield.

In addition to the consolidation hyper acute stroke care South Yorkshire and Bassetlaw are committed to ensuring high quality acute stroke and TIA care, inpatient rehabilitation, early supported discharge and community rehabilitation and the delivery of this through a network approach that values every aspect of the stroke pathway. The model also includes the gradual development of Thrombectomy services.

Finally the model is also clear that there is an on-going need to continue to focus on prevention, primary and secondary prevention and it is anticipated that the network will provide a platform for collaboration around this and linkages to other work streams in the South Yorkshire and Bassetlaw Accountable Care System to maximise the potential to improve outcomes through this across the region.

The candidates will choose which District Trust to develop services from but will have guaranteed HASU work at Doncaster and Bassetlaw Teaching NHS Foundation Hospital Trust. This recognises that the development of stroke services in the future will require working collaboratively across the network and is pioneering these roles for the future.

The role offers the exciting prospect of covering the HASU at Doncaster Royal Infirmary. Work on HASU replaces the normal working week and would occur 6 weeks per calendar year (including prospective cover). The appointee would be responsible for assessing all acute admissions, including making decisions regarding suitability of patients to receive intravenous thrombolysis and thrombectomy. In addition, the appointee would be responsible for supervising the daily transient ischaemic attack clinics. CPD and MDT support will be available through monthly attendance at the neurovascular MDT and thrombolysis meetings. This is essential for professional development needs and to continue the networking ethos.
