An emergency care centre with a dedicated paediatric unit and a specialist centre for planned care in a brand new hospital. As part of our investment in the future we have a number of Consultant posts that we are recruiting to. We are recruiting three consultant physicians/geriatricians. The Trust has recently undergone a major Health and Social Care review involving commissioners, partner organisations, Local Authority and Primary Care.

This has led to the exciting programme of developing “Care Closer to Home” with specialist hospital care being delivered at or near the patient’s home. Examples include Quest for Quality, a newly commissioned service in Calderdale. This is a geriatrician lead specialist service for residential and nursing homes in Calderdale. The consultant geriatrician leads a team of community based matrons working in residential care to improve the medical care of patients and reduce hospital admissions. This is a geriatrician led specialist service for residential and nursing homes in Calderdale. The consultant geriatrician reviews patients where necessary and supports a weekly MDT. Care Closer to Home will involve further community based geriatric services being developed

The team also takes an active role in managing acute emergencies and spends a significant part of their time in a proactive prevention role, for example, advanced care planning or medication reviews. The trust has joined the national acute frailty network and we are developing comprehensive geriatric assessment for frail patients who present as an emergency in a team with acute therapists, community in reach nurses and social services. Joining us at this time will give you the opportunity to shape and develop these services.
