Our Consultant Geriatrician’s are currently leading the way in caring for older people in crisis, and through the Frailty Pathway being delivered at Basingstoke and North Hampshire Foundation Trust we are looking to recruit enthusiastic consultants to support our future vision of providing quality care of older people with Frailty, reducing length of hospital stay and ensuring safe and sustainable discharge. We have been successful through working
with the Acute Frailty Network at reducing length of stay, working to get the right patient in the right place at the right time.

You will be joining a department at Basingstoke hospital with a commitment to service improvement; good integrated working with community services, with locality based geriatricians working both in the acute trust and with GPs and community teams. We want to develop this further by addressing the pathway frail older patients take when they reach crisis. Our Frailty service includes the development of an Acute Frailty Unit at Winchester, and a multi-disciplinary team (integrated frailty intervention team – IFIT) based in ED and
AAU on the Basingstoke site. Both sites already have rapid assessment clinics and are looking to develop these ambulatory services as part of our drive to improve frailty pathways and integration with community services.

There will be a commitment to your successful integration into the team, though a culture of supportive working and mentorship.

This post will be predominantly based on the Basingstoke site, where all our Frailty services are located on F Floor across 3 wards. Rotational working through Basingstoke and Winchester site would be considered, as well as patterns that support flexible working.

On-call commitments are on the acute medicine rota for which an additional 2 PA’s for on call duties). There is a weekend ward round rota approximately 1:12 for morning ward round seeing targeted patients on the medical wards with time off in lieu.

The applicant should hold a CCT or equivalent in General Internal Medicine and Geriatric Medicine or be with 6 months of completion.
