Applicants are invited for a newly created position at the London North West Healthcare NHS Trust.

The post holder will be an integral part of the team delivering diabetes, endocrine and inpatient general medical care to patients on behalf of the Trust. The post holder will provide consultant support to the Brent Integrated Diabetes Service team to enable and facilitate the delivery of integrated diabetes care in the Brent CCG area. This would include providing level 2/3 (NHS London Diabetes) community based multidisciplinary clinics for individuals with diabetes and in-practice virtual clinics and GP education.

The post holder will provide clinical leadership to the community based diabetes team in Brent. At Central Middlesex Hospital they would be the adult service lead for Transitional Diabetes. The post holder’s job plan will be divided with 50% of their time in the acute team and 50% in the community.

The post requires the provision of medical cover to the department both within and out-of hours and this cover is provided at Northwick Park Hospital site.
