Consultant in Palliative Medicine – Community Based
Applications are invited for the post of Community Consultant in Palliative Medicine in Hull. Working in an established team of four consultants in palliative medicine who are employed by the local Acute Trust (Hull and East Yorkshire Hospital NHS trust), the post holder will be the clinical lead for the community palliative care team provided by the local award winning social enterprise, City Health Care Partnership (CHCP).
CHCP is the primary provider of community services in Hull and has joined with the local Acute Trust to create a role that will help to ensure a seamless specialist palliative care service across hospital, hospice and community. Employed by the acute hospital and working as part of the palliative medicine consultant team, the post holder will have an NHS contract and sessions with either the acute hospital or hospice depending on the post holder’s interests, and to be agreed in team job planning.
This is an exciting opportunity for the post holder to develop integrated community services to ensure a seamless delivery of palliative and end of life care for patients in Hull. As part of the consultant team, the post holder will develop the community service and provide leadership in order to deliver integrated and patient centred palliative care medicine as part of a multi-disciplinary team. Responsibilities will include:
- leading a weekly community MDT
- provision of community clinics
- supporting the Macmillan GP Facilitators and Macmillan Community Specialist Palliative Care Team with educational initiatives in relation to Specialist Palliative Care and End of Life Care
- provision of supervision for the GPwSI working in the field of end of life and palliative care
- clinical support to the nursing team within the community
The post holder will join the Regional Specialist Palliative Medicine Consultant on call rota. This is currently a second on call rota with a frequency of 1 in 14 weeks covering Hull, York, Scarborough and Harrogate.
HEYHT are teaching partners with the Hull York Medical School. There are several undergraduate teaching opportunities. It is anticipated that the post-holder will contribute to this and an application to become a HYMS clinical tutor will be encouraged and supported.
Hull, City of Culture 2017, is a thriving University City with excellent shopping and leisure facilities including concert halls, theatres and Europe’s largest aquarium, The Deep. It is surrounded by attractive villages and countryside with the minster town of Beverley nearby. It is within easy reach of the Yorkshire Moors and Dales and the East Coast. It has excellent road and rail links with the rest of Britain, as well as ferry and air links to Europe.
