Consultant Physician in Medicine for the Elderly and General Medicine
Applications are invited for a substantive consultant in Interface Geriatrics on a full-time or part-time basis to join the Elderly Care department, based at Lister hospital in Stevenage, Hertfordshire. The post comprises 10 programmed activities (PAs)
The service provide early comprehensive geriatric assessment for older frail patients at the front door working closely with colleagues in emergency and acute medicine, along with the provision of daily rapid access clinics and a new 20 bedded frailty unit.
This is an excellent opportunity within the Elderly care department at East and North Hertfordshire NHS Trust. Successful applicants would join an established team of consultants, specialist nurses and junior medical staff to deliver comprehensive geriatric assessment and improved outcomes for older patients.
The service was commissioned by the CCG and has received ongoing funding following a successful Vanguard bid to further expand including delivering frontline and community sessions.
The posts provide an important role in bridging the links with primary and secondary care. Newly appointed colleagues would work within the department with the following key objectives:
- Prevent unnecessary decline in an older person through proactive assessment and review.
- Work with colleagues in primary care to develop robust alternative pathways of care for older persons who do not require admission at times of crisis.
- Work with Residential and Nursing homes to provide education and training around complex clinical case management as well as supporting primary care with the development of advanced care planning/end of life care.
There are opportunities within the department for successful candidates to pursue other interests including education and management as well as mentorship schemes for newly appointed consultants.
Following the investment of £150 million in major new facilities at the Lister, along with a £30 million New QEII hospital built by the local clinical commissioning group, the East and North Hertfordshire NHS Trust has brought together all its specialist emergency and inpatient services at the Stevenage hospital. Our state-of–the- art clinical facilities include a major new Emergency Department, the Hertfordshire Cardiology Centre (which includes a 24/7 PPCI service), new scanning centre, McMillan chemotherapy unit, ward, endoscopy and theatre blocks.
