'At Hampshire Hospitals NHS Foundation Trust patient care is at the heart of everything we do.'

We are looking for enthusiastic and motivated consultants in Geriatric Medicine to join our team of current consultants to help develop and deliver better pathways of care for frail older patients in our area. We are looking to reorganise the way we deliver safe and effective care to this group of patients across the whole healthcare system in Mid Hampshire.  

We already have good integrated working with community services with locality-based geriatrician working both in the acute trust and with GPs and community teams. We want to develop this further by addressing the pathway frail older patients take when they reach crisis. The first steps in our Frailty Strategy include the development of frailty units on Basingstoke and Winchester sites, increased integration with community services in this setting and further expansion of our ambulatory care assessment services. We have joined the 2016 cohort of the Acute Frailty Network and Hampshire Hospitals is committed to supporting the changes necessary to deliver the best care to frailer people.

This post will be predominantly based on the Basingstoke site, where all our Frailty Services are located on F Floor across 3 wards. 

On-call commitments are on the acute medicine rota with an additional 2 PA’s for on-call duties. There is a weekend ward round rota approximately 1:12 for morning ward round seeing targeted patients on the medical wards with time off in lieu.

The applicant should hold a CCT or equivalent in General Internal Medicine and Geriatric Medicine or be with 6 months of completion.
